Please fill out a separate entry form for each book. If you are entering the same title in multiple categories then use one form, enter the appropriate categories below and pay the entry fee ($75.00) multiplied times (x) the number of categories entered.

Please note: None of the following information will be distributed publicly.

After you’ve completed your submission form and paid for your entry, please mail one physical book for every category entered to:

Because of the high volume of entries, it could take up to two weeks to send you the notification that we received your book.
